QVAD Review Board IMS is the online platform for completing and submitting forms to the Voluntary Assisted Dying Review Board as required by the Voluntary Assisted Dying Act 2021

It is also where practitioners can submit an application to become an authorised voluntary assisted dying practitioner. 

QVAD Review Board IMS is not integrated with other clinical systems.

OUTAGE NOTICE: Ahpra Practitioner Information Exchange
You will be unable to apply to become an authorised practitioner through the QVAD Review Board IMS from 8:00am to 3:00pm AEST, Sunday 6 October 2024 due to a scheduled outage to the Ahpra Practitioner Information Exchange.

Practitioners will continue to be able to access the QVAD Review Board IMS to manage cases and submit approved forms during this time. Please email VADReviewBoard@health.qld.gov.au if you require assistance.

I need to complete a form

You do not need access to QVAD Review Board IMS to submit the
following forms. However, if you do have access please log in first.

I have received a referral for consulting assessment. 

I have received and disposed of a voluntary assisted dying substance . 

I have completed a cause of death certificate for a relevant patient. 

I want to submit a completed form
